Based in Eau Claire, Wisc., Luther Midelfort offers a full range of quality medical services, including complete cardiac surgery, cancer and trauma care programs, through a network of community-based healthcare providers in west central Wisconsin.

Credentialing
Valley Health Plan requires all providers to go through an extensive application process before they are considered a network provider. This process, known as provider credentialing, ensures that our providers can offer our members quality care and service. Some examples of sources checked during the credentialing process include history of provider's license activity, malpractice history, hospital privileges and confirmation of appropriate board certification. Physicians must complete the credentialing process and be accepted before they can see Valley Health Plan members. Making sure that our providers meet or exceed our high standards is just another example of Valley Health Plan's commitment to quality care and service for our members.
