Subscriber Privacy Policy
The company listed
above recognizes the importance of our customers’ trust. Because you and your
family have entrusted us with your nonpublic personal financial information, we
are committed to safeguarding that information. This Notice, which is required
by state and federal law, explains our Privacy Policies.
Why
Do We Collect Your Personal Information?
In order to serve your needs, there are occasions when we may collect personal
information about you. We collect personal information from you to help
us:

Process your enrollment application;

Pay claims; and
-
Coordinate benefits with other insurance companies.
What
Type of Personal Information Do We Collect?
We treat any information that is identifiable to you and not available to the
general public as your personal information. For example, we may collect
information we receive from you on an application for insurance, or information
from your transactions with us, our affiliates or others, such as:

Your name, home address, telephone number, date of birth, social security
number;

Employer name and income, when necessary for coverage;

Information regarding your insurance plan, such as type of insurance coverage
and benefits; and

Premium payment history.
We may also collect personal information about you from your health care
providers or other participants in the health care system, as well as from your
employer or plan administrator, credit bureaus or consumer reporting agencies
to process claims or to approve your application for coverage.
What
Types of Personal Information Do We Disclose?
The personal information we collect is used to administer your plan
effectively. We will not disclose your personal information unless we are
allowed or required by law to make the disclosure. Following are some
categories of disclosures we may make as allowed or required by law:

We may disclose all of the information we collect to our affiliates that offer
insurance and related services and products, such as subrogation.

We may disclose all of the information that we collect to service companies
(both affiliates and non-affiliates) that perform insurance functions on our
behalf, such as insurance agents, auditors, benefit consultants, or care
management specialists for utilization management and quality improvement.
(When we do this, we require them also to protect customers’ personal
information.)

We may disclose all of the information we collect to health care providers
(doctors and others who provide you with care) in connection with an insurance
transaction, such as verifying that you have coverage.

We may disclose all of the information we collect to an insurance regulatory
authority, such as the State of Wisconsin Office of the Commissioner of
Insurance.
Opting
Out of Information Sharing
You may have received notices from other organizations that allow you to "opt
out" of certain disclosures. The most common type of disclosure that applies to
"opt outs" is the disclosure of personal information to a non-affiliated
company so that company can market its products or services to you. As a health
plan, we must follow many federal and state laws that prohibit us from making
these types of disclosures. Because we do not make disclosures that apply to
"opt outs," it is not necessary for you to complete an "opt out" form or take
any action to restrict such disclosures.
Safeguarding
Your Personal Information
We maintain physical, electronic and procedural safeguards that comply with
applicable federal laws and regulations to guard your personal information.
Upon your request, we will assign a random identification number as your health
plan subscriber number, instead of using your Social Security number. We also
restrict access to your personal information to those employees who need to
know the information in order to perform their job duties, and provide them
with regular training on confidentiality. Any employee of the companies listed
in this Privacy Notice who would violate our privacy policies would be subject
to discipline.
